Reccurent urinary tract infection

Rekurentne infekcije mokraćnog sustava odnose se na pojavu tri ili više epizoda u godinu dana dvije unutar šest mjeseci, a koje su dokazane urinokulturom.Česte žena svih dobnih skupina, te tako oko 60% ima barem jednu epizodu cistitisa tijekom života, procjenjuje da će 20-40% imale cistitisa, vjerojatno imati još jednu, od kojih 25-50% rekurentne infekcije. Predispozicijski čimbenici jednaki kao i za pojedinačne epizode, ponešto razlikuju generativne dobi onih menopauzi. Glavni predispozicijski čimbenik mladih je spolni odnos, postmenopauzi nedostatak estrogena. Nakon provedene terapije indiciran neki oblik antimikrobne, potom neantimikrobne profilakse, prenosimo smjernice liječenje profilaksu. U Ambulanti urogenitalne Klinike infektivne bolesti "Dr. Fran Mihaljević", Zagreb, razdoblju 1.10.2021. do 30.9.2022. bilo 996 pregleda, radi rekurentnih infekcija pregledano 275 kroz 503 pregleda. Njih 87,3% imalo jedan čimbenik, najčešći bili peri- postmenopauza, inkontinencija mokraće. Antimikrobnu profilaksu primalo 93,1% bolesnica, najčešće trajanju 6 mjeseci (31,3%) to nitrofurantoinom (72,3%), fosfomicinom (11,7%), sulfametoksazol/trimetoprimom (9%). Neantimikrobnu uzimalo 26,5% žena, D-manozu (91,8%). Dokazani recidiv vrijeme nakon profilakse 16,7% njih 32,6% uzročnik bio isti primarne S obzirom učestalost mokraćnoga sustava, važno ispravno postavljanje dijagnoze, indikacije antimikrobnu profilaksu, kako bi poboljšala kvaliteta života bolesnica. No, s rastuću rezistenciju mikroorganizama antibiotike, treba biti racionalan njihovoj primjeni profilaktičke svrhe. Stoga izbor bolesnica njihovo praćenje ključni. Za postupanje svakodnevnom kliničkom radu potrebno uvijek pratiti najnovije stručnih društava.
